Patient Information

New patients are always welcome. On your first visit to the clinic, the receptionist will assist you in establishing a medical record. Creekside Medical Clinic strictly adheres to HIPAA Privacy Guidelines and respects confidentiality of your medical records and of private information shared during your medical appointment. Patient Registration

We value your time and ask that you print and complete the new patient registration form. Click here to download the form also download the Medical History Form. Fill these forms out prior to your first appointment in order to save time when you check in.

Sports Pre-Participation Exam

Please click here to print the two page form for sports physicals. A parent or legal guardian must fill out and sign the history questionnaire prior to the child's appointment.

DOT Physicals

Steve Sachs, PAC is certified to perform DOT physicals. Please click here to print out the DOT form. You will need to complete this and either fax it back to our clinic at 605-341-3552 or bring it with you on the day of your appointment. We are unable to see you for a DOT physical without this document completed prior to your visit.

Telephone Calls

You are welcome to call the clinic with medical questions about yourself or another clinic patient for which you are a parent or guardian. We make every effort to return phone calls by the end of the business day. Please schedule an appointment for medication requests, medication changes, and for any illness or symptom that may require treatment.


Please call Monday – Friday, 7:30 am – 5:00 pm to schedule an appointment.

We try to schedule same-day appointments for urgent medical conditions or illnesses. Please communicate with the receptionist the urgency of your appointment request so we can accommodate your needs.

If you cannot keep your appointment, please call and cancel at least 24 hours in advance so we may give that time to another patient.

Patients who frequently fail to show for scheduled appointments without calling to cancel are asked to find another medical provider.

Medication Refills

As a rule, you should always speak to your pharmacist about refills prior to calling the clinic. Do not rely on the pharmacy’s automated phone system- speak to a person.

Expect to schedule an appointment if you have run out of refills because this is an indication you are overdue for a regular follow up appointment for your medical condition.

If you call for medication refills after 2:00 pm, we may not be able to accommodate your request until the next business day.

After Hour Emergencies

Our physicians do not offer after hour emergency phone or medical care. Please call 911 if you are having a medical emergency.

If your medical concerns cannot wait until the next business day, there are urgent care clinics and an emergency room in Rapid City that offer extended hours. We will gladly see you in follow -up and obtain your records as needed.
