We value your time and ask that you complete the below forms. Filling out these forms prior to your first appointment will save time when you check in. When you have completed this form you may either print the form and bring it with you or you may save the file to your computer and email it to our front office supervisor at admin@creeksidemedicalclinic.org. Please know that you will have to physically sign the form when you come into the office as per HIPAA regulations all forms need a physical signature.

Please print and fill out the two-page form below for sports physicals. A parent or legal guardian must fill out and sign the history questionnaire prior to the child's appointment.
Sports Physicals Form
Steve Sachs, PAC is certified to perform DOT physicals. Please download and complete the DOT form below. You will need to complete this and either fax it back to our clinic at 605-341-3552 or bring it with you on the day of your appointment. We are unable to see you for a DOT physical without this document completed prior to your visit.
DOT Physical Exam Form
You are welcome to call the clinic with medical questions about yourself or another clinic patient for which you are a parent or guardian. We make every effort to return phone calls by the end of the business day. Please schedule an appointment for medication requests, medication changes, and for any illness or symptom that may require treatment.
Please call Monday – Friday, 7:30 am – 5:00 pm to schedule an appointment.
We schedule same-day appointments for urgent medical conditions or illnesses. Please communicate with the receptionist the urgency of your appointment request so we can accommodate your needs.
As a rule, you should always speak to your pharmacist about refills prior to calling the clinic. Do not rely on the pharmacy’s automated phone system- speak to a person.
Expect to schedule an appointment if you have run out of refills because this is an indication you are overdue for a regular follow up appointment for your medical condition.
Our physicians do not offer after hour emergency phone or medical care. Please call 911 if you are having a medical emergency.
